In compliance with 45 C.F.R. 164.520, this notice explains how your health information may be used and disclosed, and how you can access this information.

Ching H. Wu keeps your health information in a patient record that is the property and responsibility of Ching H. Wu.

He uses the information to provide acupuncture and TCM herb treatment and facilitate administration. It is kept in strict confidentiality and will not be disclosed without your written consent except for that required by laws.

You are welcome to have a copy of this Notice of Privacy Practices any time.

You may request access to your health record in writing. If you want a copy of your health record, a fee covering the copying and associated costs may be charged to you.

You may request for an accounting of your health information for insurance, tax and other uses.

You may complain to Ching H. Wu or to the Department of Health and Human Services if you feel your privacy rights have been violated.

Ching H. Wu may disclose your health information as required by federal, state, or local laws.

Ching H. Wu may use your health information in a manner not identifying nor revealing you.

Ching H. Wu reserves the right to change this Notice of Privacy Practices according to the laws.

Under the Federal HIPAA Privacy Rule, a copy of this Notice of Privacy Practices must be provided to you before Ching H. Wu can provide acupuncture and custom-made herbal treatments. By signing the Consent to Treatment by acupuncture, Qi-Gong Exercise and/or TCM Herbal Formula for Ching H. Wu, you acknowledge the receipt of this Notice of Privacy Practices.
