New Patient

1. Read the Privacy Policy and Release Form. Understand and consent to the Privacy Policy and Release Form.
2. Fill out the form below and pay for the e-diagnosis (TCM), $60.00 which covers the initial registration, differential traditional Chinese medicine diagnosis analysis and a report on the e-diagnosis results. (The questionnaire will be sent to your email address.)
3. Answer the questions in the questionnaire as much as possible and email the questionnaire to Dr. Wu.
4. Dr. Wu will go over the questionnaire, make traditional Chinese medicine differential diagnosis to identify the root cause of the problem and report the diagnosis results and recommendations by email.
5. If you decide to purchase the custom-made traditional Chinese medicine herbs for your current conditions, you need to go to the Repeating Patient page. The TCM herbs are $45/week plus $5 S&H.
